Additional Information

Description

This pediatric scenario addresses learning objectives applicable to all types of undergraduate nursing programs. The scenario is designed to promote development of patient assessment skills, identification of nursing diagnosis, management of care, and implementation of appropriate interventions.

The scenario presents Michael Smith, a 6-year-old Caucasian male who is now 5 days post-admission to the Pediatric Intensive Care Unit with a severe skull fracture after a fall from the bed of a pick-up truck driven by his father. A massive brain injury secondary to a severe basilar skull fracture and a dural tear has resulted in a terminal state and brain death has been established. Recently the decision not to resuscitate (DNR) has been made, the parents have agreed to organ donation, and the organ procurement organization has been involved. The students are expected to perform a full neurological assessment and to discuss the prioritization of nursing care. They are also expected to demonstrate effective communication with family, identify potential family resources (chaplaincy, social work), consider the family’s Baptist beliefs relating to end-of-life decisions, communicate plan of care to parents, and implement comfort measures for both patient and family.

For ease of use and to support the integration of the scenarios into curricula, the following features are included in the scenario:

• A list of the 2016 NCLEX-RN Test Plan Categories that are being addressed in the scenario
• A list of the nursing diagnoses that the students should be able to identify during simulation
• Guided reflection questions mapped to the Quality and Safety Education for Nurses (QSEN) competencies for use during the debriefing
• Appropriate documentation and order forms to help enhance realism

The scenario is developed for use with the Laerdal SimJunior Advanced pediatric simulator. Together, this scenario and the SimJunior simulator offer faculty the ability to provide realistic and challenging scenario based simulation to help students apply the nursing process. The scenario is also available as part of the set “Simulation in Nursing Education – Pediatric Scenarios”.
